Three Questions About Salat

As-Salam Alaikum,

I have a couple of quick questions that I hope somebody can help me with. I would also really appreciate if I could get at least 2 responses just to make sure that what was answered is definitely correct (since I would like to learn things right from the beginning :) )

1. During Salat do I say Ameen after Surah Al-Fatiha when praying alone, or is Ameen said only when praying in congregation after Imam has finished reciting?

2. When praying together with my wife, and I take the Imam's role, do I say Ameen after i finished Al-Fatiha or only she (the one who follows)

3. When I finish Al-Fatiha I like to pray Surah Al-Falaq as my second Surah. Do I start again with Bismillaah ar-Rahman ar-Raheem or with Qul-a'udhu bi Rabb il-falaq? Basically the question is do I start the second Surah with Bismillaah ar-Rahman ar-Raheem as well (since I am not sure if Bismillaah ar-Rahman ar-Raheem is the integral part of the Surah) and should it be recited regardless if it is or not the part of the second Surah?
